Treatment by a doctor is also a matter of trust. In addition to professional qualifications, for many people interpersonal factors also play a role in choosing a doctor. 

But under what circumstances are you free to choose your doctor? What should you look out for? And when do you need supplementary insurance to allow you to choose your doctor? You can find out about all of this in this article.

Free choice of doctor as a fundamental right

Free choice of doctor means that insureds have the option of choosing their own doctor. You can decide which healthcare professionals you want to see – and seek treatment from a doctor who you think best meets your needs. The free choice of doctor does not only affect which actual doctor you see: You are also free to visit a specific hospital or clinic. This is particularly important for complex or specialized medical procedures. 

In Switzerland, the free choice of doctor is governed by law, or more precisely by the Federal Act on Health Insurance (KVG/HIA). In terms of a rough summary, the following points apply:

  • Under compulsory basic insurance, you are essentially free to choose your doctor. This freedom of choice applies both to outpatient treatment and to inpatient stays in hospitals that are on the hospital list of the respective canton. However, the scope depends on the health insurance model you choose: Under the standard or traditional model, you always have a free choice of doctor. This makes this model the most flexible, but often also the most expensive. Find out more about the other models below. 
  • If you have taken out supplementary insurance, you can often benefit from extended options in terms of the free choice of doctor. Some supplementary insurance options make it possible, for instance, to be treated in private hospitals or by certain specialists. This insurance often also allows access to special treatment methods and shorter waiting times.
  • In emergencies, if immediate medical attention is required, you always have the choice of the nearest suitable medical practice or hospital.

What applies to cross-cantonal treatments?

If you would like treatment in a canton other than the one in which you live, this is possible. This applies in particular to special treatments or if you prefer certain doctors who are not located in your home canton.

Different models for choosing a doctor

Your health insurance fund or your insurance model plays a decisive role in the free choice of doctor. In addition to the standard basic insurance model, there are the following:

General practitioner model

Under the general practitioner model, you undertake to always consult your general practitioner first if you have any health problems. They then coordinate all further treatments and referrals. You are free to choose your general practitioner’s practice, but you must seek recommendations from there for all other medical services. However, the current shortage of general practitioners can limit this freedom of choice, as you first have to find a general practice that will accept you as a new patient.

HMO model (Health Maintenance Organization)

With the HMO model, you choose a specific doctor’s practice or healthcare organization that will take care of your primary health care as the HMO center. All treatments must be carried out through this center, and only the doctors working there may treat you. Lower premiums can be calculated for this model.  

Telmed model

With the Telmed model (from “Telemedicine”), you must always first call a medical hotline before visiting a doctor. The advisors will then give you recommendations and take care of any referral. You cannot go to a doctor’s practice of your choice directly without prior telephone advice – except in an emergency.

Can I have two different general practitioners at the same time?

It is generally not possible to have two general practitioners at the same time. In emergencies or in the event of special medical problems, you can consult other doctors or specialists. However, you should discuss these visits with your general practitioner.

Other factors that are important in the free choice of doctor

In addition to the insurance model, other factors can also restrict the free choice of doctor. Some insurers and cantons, for example, set budgets or cost limits for certain treatments. These financial conditions influence which doctors and hospitals you can visit at no extra cost.

In addition, the choice of hospital may also depend on the cantonal hospital lists. Each canton maintains a list of recognized hospitals for which basic insurance covers the costs. If you choose a hospital that is not on this list, you may have to pay the costs yourself. An exception is if it involves specialized treatment that is only available outside of your canton.

Thanks to the free choice of doctor, you can also consult doctors who are particularly qualified for your specific health problems, should this prove necessary.

This, in turn, has an impact on patient satisfaction. Ultimately, freedom of choice allows you to find healthcare professionals who better meet your individual needs and expectations. Be it in terms of treatment methods or communication.

Free choice of doctor, fine – but what's the best way of finding a doctor that suits you and your individual needs? Try these tips:

1. Research online

Use review portals and websites of medical practices to find information about doctors near you. Read testimonials from other patients to get an idea.

2. Ask friends and family

Personal recommendations are very valuable. Ask people around you about good experiences they have had with specific doctors. This can help you find trustworthy and competent medical support.

3. Check the qualifications

Make sure that the doctor has the necessary qualifications and expertise for your specific health needs. Information on this can often be found on the website of the practice or on the websites of medical associations.

4. Pay attention to availability

Make sure the practice is easily accessible and the opening hours are in line with your schedule. A practice close to your place of residence or your place of work can save you a lot of stress and time in your everyday life.

5. Trust your gut feeling

Personal impressions are important. If you do not feel at ease, do not hesitate to look for another doctor. Your well-being and confidence are key to successful treatment.

6. Clarify the invoice

Ask how services are invoiced, especially if you have supplementary insurance. Clarify what costs your health insurance will cover and whether there are additional payments.
