Do you need a specific form, information sheet, or other document relating to your pension fund? We have put all the documents on your pension fund’s website for you.

Where can I see which pension fund I’m insured with?

  • As an insured member you can find the name of your pension fund on your pension fund certificate. You can view your personal certificate easily at myAXA under “Documents”.
  • Employers can find the name of their pension fund on each customer letter or in their affiliation contract. Tip: It’s now even easier to submit change requests and applications via “OPA Services” in myAXA.

Tips for faster processing

  • To ensure fast processing, please always use the most recent version of each form rather than stockpiling printouts or saving empty forms for later use.
  • To enhance legibility you can complete the forms directly on screen and then print them out or send them directly.
